Rhinogram: Copying Consent Forms to Minor Patients' Records

When a consent form is sent to a minor’s guarantor for completion, a user will need to manually write the completed form back to the minor patient’s attachments in axiUm. (This process is necessary only for minor patients; it is automated for adult patients.)

The instructions below assume the form has already been completed by the patient’s guarantor.

Instructions

  1. Select the minor patient in axiUm, then click RG Messenger.
  2. The right side Summary panel will show the guarantor under Connected Parties. Click on the guarantor’s name. (If no connected party shows in Rhinogram, then no guarantor is listed in the patient card in axiUm.)

  3. In the guarantor’s Summary panel, click “View” next to Library, then click Completed Forms. Select the form you wish to write back, then click Copy To. A new window appears.
  4. Select the patient record to which you want to copy the form, then click Copy Now.

  5. Navigate back to the patient’s profile by clicking their name, which will appear in blue with the message "Copied to [Patient Name's] Library."
  6. The copied consent form will appear in the patient’s Library under Completed Forms. Click the completed form, then click Copy To. A new window appears.
  7. Check box to copy PDF to EHR, then click Copy Now to send the form to the patient’s Attachments in axiUm.

Who can resolve this problem? Any user with Rhinogram access can resolve this.
